Clinical

Nurse Practitioner -Part time will considerFull time - Covering Georgetown University - Full time- Washington, DC covering DC area hospitals

The Nurse Practitioner is an advanced practice nurse who provides direct care and clinical expertise to individuals, families and groups in the hospital setting.  In collaboration with the physician and other staff, she/he is responsible for the assessment, planning, implementation and evaluation of palliative care interventions and providing follow up for patients during phases of wellness and illness. Work is performed with general independence and decision making responsibility in accordance with palliative care clinical guidelines and scope of practice, and the patient's attending physician.

Admission RN       WP

The Admissions Nurse, in accordance with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families, in a collaborative relationship, with other members of the interdisciplinary team. The Admission Nurse makes initial nursing assessments, judgments and treatment decisions based on patient/family needs and wishes in accordance with professional skills.

The Certified Nurses Aide/Home Health Aide provides direct patient care within the homecare or assisted living environment.

RN Coordinator                  WP

RN Coordinator provides the primary coordination of hosptial based hospice and palliative care. The RN Coordinator receives all hospice and palliative care referrals and arranges for admission to the hospice or palliative consultative service, interfaces with the patient/family, and with the attending physician. The RN Coordinator triages all incoming referrals and coordinates interdisciplinary team members required to best meet immediate needs of patient. In collaboration with the hospice and palliative care medical consultant (when applicable), the RN Coordinator initiates the care plan and discharge planning and communicates with the interdisciplinary team regarding patient goals and treatment plan.

Requirements:

* RN Licensure from the District of Columbia to practice professional nursing.

* BSN preferred.

* Minimum of two years of public health/community health, medical surgical or hospice nursing experience.

* Hospital based nursing strongly preferred.

* Strong communication and counseling skills of added benefit.

* Certifications in Hospice and Palliative care a plus.

(2) Full time- Washington, DC

RN Case Manager       WP

The RN Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers. Working in collaboration with other memebers of the interdisciplinary team, the RN Case Manager plans and manages the nursing component of the hospice caseload. The RN Case Manager makes nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs and wishes and in accordance with her/his professional skills.

Under the supervision of a Registered Nurse and in accordance with professional nursing standards and practices, the Licensed Practical Nurse provides, teaches, and supervises nursing care for Capital Hospice patients requiring Hospice Care.

Extended Care & On- Call RN               WP

The Extended Care RN provides, arranges for, teaches and supervises nursing care of Hospice and Home Health patients/families, in a collaborative relationship, with other members of the interdisciplinary team. This position plans and manages the nursing component of the home health caseload. The Extended Care Nurse makes nursing assessments and judgments based on patient/family needs and wishes and in accordance with professional skills.

Chaplain                                                           WP

The Chaplain develops the spiritual component of Hospice care in accordance with the patient's and family's needs and desires and delivered in accordance with the overall plan of care. The chaplain miisnters to the spiritual needs of Hospice patients, families, caregivers and staff.

Requirements:

* Any of the following are desirable:

LPC or similar license; four certified units of clinical pastoral education in an accredited hospital (or the equivalent of such education); professional certification as a chaplain by national clergy certifying organization; ecclesiastical edsorsement for chaplaincy by the faith community.

* Master of Divinity dergee from an accredited professional theological school or semnairy involving at least three years of post-bachelor education, or an equivalent graduate degree with professional pastoral counseling training, ordination or official ednorsement by the candidate's faith community or the candidate's ministry.

* Three years pastoral or pastoral counseling work; experience in working with families and caregivers facing death and bereavement.
